Life Line’s Billing Policy is that all accounts are due and payable upon receipt.  Ambulance charges are the financial responsibility of the patient and/or responsible party.  Third-parties are billed as a courtesy.  A charge of 18% per annum or 1.5% per month will be applied to all accounts 30 days past due.

Ambulance Rates:   Life Line Ambulance rates are regulated by the Arizona Department of Health Services (ADHS) under A.R.S. 36-2239.  These rates are based on cost for 24-hour/7 days a week availability of emergency medical services to our communities.  To review current rates of ambulance companies in the state of Arizona, go to

LifeCare Members:  If you have received a bill in error, please call your patient representative immediately and we will make the necessary changes on your account.

AHCCCS Patients:   Please contact our office within 72 hours of the date of service so that Life Line can obtain a notification number for your transport.  This will ensure eligibility for payment.

Medicare Patients:   Please contact our office immediately with your Medicare number.  Your patient representative at Life Line will file your claim with Medicare.  You will be responsible for the co-pay and any deductible not met when transport is medically necessary as outlined by CMS.

Insurance Patients:  Please contact our office immediately with your insurance information.  Life Line will bill your carrier as a courtesy.  All co-pay, deductibles, charges not covered will be payable upon receipt of invoice after payment or denial is received.

VA Patients:  If you are a veteran, you may be eligible for transportation benefits provided by the VA.  The VA will only evaluate eligibility if the patient submits the invoice to them personally.  You can contact the Travel Coordinator at the Prescott VA at (928) 717-7523 and request that your invoice be considered for payment.

Financial Assistance:  Some patients may be eligible for financial assistance with medical bills through Arizona’s Medicaid program called AHCCCS (Arizona Health Care Cost Containment System). To contact this program for eligibility call 800-654-8713.

Credit/Debit Cards: Credit Card payments can be made online by clicking here or by contacting our office at the numbers at the top of this page.  We accept VISA, MASTERCARD, DISCOVER and AMERICAN EXPRESS.
