LifeCare Waiting List

Emergencies happen to everyone. When illness or injury strikes, strike back with a LifeCare membership.  The LifeCare Membership Plan is a program approved by the Arizona Department of Health Services that Life Line Ambulance offers to provide unlimited medically necessary* emergency and non-emergency ambulance transportation for you and your family** within our service area*** with NO additional out-of-pocket expenses.  The cost is only $49.71 per year, per family!

*Must meet medically necessary guidelines as defined by CMS (Medicare).  Transport must originate from Life Line Ambulance service areas.

**Covers immediate family, under 21-years-old, living in member's residence.

***Service area is defined by the Arizona Department of Health Services.

Enrollment is only available from September 1 – 30 each year.
