Eligibility:  All employees
When Eligible: Six (6) months from date of employment
What You Receive:  Retirement plan with employer contribution


Section 125 Cafeteria Plan

Eligibility:  All employees
When Eligible: 1st of the month following three (3) months of full-time employment
What You Receive:  Allows medical, dental & vision insurance premiums to be withheld pre-tax.

Eligibility:  Employees regularly scheduled 36-hours per week or more
When Eligible: 1st of the month following three (3) months of full-time employment
What You Receive:  PPO Plan with in-network deductible or $500 or $1,000.  includes medical, hospital, prescriptions, mental health, home health & rehab.
