Cheryl Smith
CEO/President
Cheryl Smith has been an owner and manager of Life Line Ambulance since 1978. She became the sole owner in 1997 which she became President/CEO. She has an extensive background in business management and has owned several other successful businesses. Cheryl has been instrumental in providing the vision for the company and expanding the company since her appointment as president.
She has served on the board of directors for the Arizona Ambulance Association, Northern Arizona EMS Council, as a member of Arizona Department of Health Services ADHOC committees, Yavapai College EMS Advisory Committee and Yavapai Regional Medical Center Prehospital Care Committee. She has been past chairperson for Toys for Tots, Prescott Frontier Days and Yavapai College Booster Club.
Cheryl enjoys spending time with her family, gardening and boating with her husband Jim. She has a son and daughter-in-law, Hank and Danielle, and a daughter, Brooke.
Glenn Kasprzyk
Chief Operations Officer
Glenn Kasprzyk, Chief Operations Officer, joined Life Line Ambulance in 2006. Prior to his service with Life Line, he gained emergency medical services experience as a dispatcher, paramedic, dispatch manager, and operations manager for private ambulance services in Buffalo, NY and Orlando FL.
He is responsible for and directs all day-to-day dispatch, fleet, IT and ambulance service activities.
Besides his day-to-day involvement with Life Line Ambulance, Glenn is a board member for the Arizona Ambulance Association, on the council for Northern Arizona Emergency Medical Services (NAEMS), and has presented at State and national EMS conferences on the importance of innovation and best practices in ground ambulance service.
Glenn graduated from Western New York Emergency Medical Services Institute in 1994.
Glenn is an avid sports fan and enjoys spending time at live sports events with his wife Jen. They enjoy traveling and experiencing new places.
Kristen T. Hooke
Chief Financial Officer
Kristen Hooke joined Life Line Ambulance in February of 2008 and serves as Chief Financial Officer. Kristen has responsibilities for the financial affairs of the company, including budgeting, internal and external financial reporting, tax compliance and internal controls. Kristen possesses over twenty-five years of experience serving in the financial arena. She spent the first years of her career working in public accounting, focusing primarily on financial statement review and corporate income tax preparation. She transitioned to the private sector as a Financial Manager for a publicly traded escrow and title company operating in 3 states where she was responsible for the day-to-day financial operations, as well as being very involved in SEC and Department of Insurance reporting. She spent much of her time traveling and meeting with branch managers to discuss financial benchmarks and results of operations.
Kristen holds a diploma in accounting from Albuquerque Technical Institute and a Bachelor of Science Degree in Accounting from the University of Phoenix.
Kristen enjoys spending time in the outdoors and traveling with her husband and daughter.
Jacque Holladay
Chief Administrative Officer
As Chief Administrative Officer, Jacque efficiently handles the administrative functions of Life Line Ambulance. Human Resources, Risk Management/Worker’s Compensation and CAAS Accreditation are among her areas of focus.
Jacque is an Arizona native and brings over 30 years of administrative experience to Life Line. She began her career at the Arizona legislature and transitioned to the ambulance industry in 1994. She enjoyed working with ambulance providers throughout Arizona during the 10 years she acted as Executive Director to the Arizona Ambulance Association. She has been honored for her work with various state ambulance committees and is respected for her attention to detail and organizational skills. Jacque is a member of SHRM (Society for Human Resource Management) and is certified as a Professional in Human Resources (PHR).
Jacque credits her family for providing her with extensive skills in human relations. She takes great pride in her own 5 children and the 6 additional she gained when she married her husband, Rob. With 22 grandchildren, she’s never lacking for something to do on weekends!

Edward Armijo
Chief Compliance Officer
Edward Armijo, Chief Compliance Officer, joined Life Line Ambulance in June, 2011. He is responsible for overseeing and managing compliance issues within the organization to ensure strict adherance to regulatory requirements as well as compliance with internal policies and procedures.
Prior to joining Life Line, Edward retired from the Arizona Department of Health Services where he served for 23 years. His most recent appointment was as a Deputy Chief of the Bureau of Emergency Medical Services & Trauma System. In his former capacity with the Bureau, Armijo oversaw and supervised the ground ambulance Certificate of Necessity (CON) program, air ambulance licensing, EMT certification program, base hospital licensing, EMT training programs and trauma facility designations.
Edward and his wife live in Phoenix with two of his three daughters. Edward is an avid golfer who enjoys playing golf anytime, anywhere with anyone.
Peter Como
Operations Manager
Peter Como, Certified Emergency Paramedic, began his career at Life Line Ambulance in May of 2002. He has served as a Field Training Officer, Crew Leader, and Supervisor, until becoming the Operations Supervisor, and later the Operations Manager.
Peter is a Nationally Registered Paramedic certified in BLS, ACLS, PALS, GEMS, ITLS, AMLS, and EVOC. Courses completed include National Incident Management Systems, Fitch & Associate’s Ambulance Service Manager Program, NAAC Ambulance Service Coder program, and the Emergency Medical Dispatcher program. Peter serves as Life Line’s Training Center Coordinator for the American Heart Association and is a certified instructor for Basic Life Support, Advanced Cardiac Life Support, Pediatric Advanced Life Support, Geriatric Education for Emergency Medical Services, and Emergency Vehicle Operations.
Peter is currently responsible for managing Life Line Ambulance’s field operations for Williams, Wickenburg, and Bagdad regional operations. He is actively involved in the community as a volunteer and enjoys spending time with his wonderful wife and two children.
Scott Keilman
Operations Manager
Scott Keilman, Operations Manager, has been with Life Line since 2003. He is responsible for the day to day operations of the Prescott region, which includes Prescott, Prescott Valley, Chino Valley, Dewey, Humboldt and Mayer. Scott works closely with the Field Supervisors to provide quality assurance feedback to the field staff as well as working closely with local First Responder agencies to help provide the best patient care.
Scott is an Arizona Certified Emergency Paramedic with additional training in Advanced Cardiac Life Support, Pediatric Advanced Life Support, CPR, Emergency Medical Dispatching, and Certified Ambulance Coder.
Scott grew up in the Prescott area and graduated from Prescott High School in 1990. He then went on to an excellent career in the U.S. Navy until April of 2000. Scott then returned to Prescott in November of 2000 and enjoys a wonderful life with his wife, Tracey.
Terri Morales
Accounts Receivable Manager
Terri Morales joined Life Line in 2005. Terri has over 20 years administrative experience in health care. She has held many positions at Life Line and is currently our Accounts Receivable Manager, and has been since 2007. Her range of experience comes from working for an insurance company processing claims, providing customer service and learning the complexities of underwriting.
Her responsibilities at Life Line include managing the Accounts Receivable staff that process the billing of any third-party payers, including Medicare and AHCCCS. Terri is also responsible for HIPAA and DHS billing compliance, patient customer service and Life Line’s annual LifeCare Subscription program. Terri is a Certified Ambulance Coder, has attended numerous workshops and conferences specializing in ambulance reimbursement and is currently working on her management certification at Yavapai College.
Terri has lived in Arizona for a little over 5 years with her husband and son, relocating from Southern California. They enjoy traveling, boating, sports and the company of family and good friends.
